The Colorado Beacon Consortium is made up executive-level representation from four mission driven, not-for-profit, Western Colorado-based organizations, all of which have a nationally acknowledged track record of coordination to achieve superior outcomes. They include Mesa County Independent Physicians’ Practice Association, Quality Health Network, Rocky Mountain Health Plans and St. Mary’s Regional Medical Center. A representative from each organization sits on the Colorado Beacon Consortium’s Executive Committee, along with a consumer representative-at-large.

Our Mission
The Colorado Beacon Consortium’s mission is to optimize the efficiency, quality, and performance of our health care system, and integrate the delivery of care and use of clinical information to improve community health. The geographic focus of the Consortium’s activities includes the Colorado counties of Mesa, Delta, Montrose, Garfield, Gunnison, Pitkin, and Rio Blanco.
The Colorado Beacon Consortium (CBC) has been awarded $11,878,279 by the U.S. Department of Health and Human Services to participate in a 3 year health care demonstration program to expand the Quality Health Network (QHN) services and to provide a Practice Transformation Team including Quality Improvement Advisors (QIA) to work with medical practices.
Practice Transformation is improving care coordination and care transition to increase efficiencies within medical practices thereby supporting good health for all patients in the community. The CBC takes Practice Transformation to the next level by working with medical practices to improve the quality of care by helping them to maximize the tools and staff they already have while considering the practice’s patient population and surrounding community.
A medical community committed to quality and efficiency builds a healthier community as a whole, which will equate to lower overall costs, to patients, employers, and taxpayers. It’s estimated that 300,000 people will be positively impacted by this program.
