What does this mean for you and your practice?
In the next few weeks, you will be working with CMS and participating payers to finalize program contracts and expectations. Contracting arrangements will be finalized by 11/1/2012 which is the kickoff for the program and begins the first month for which enhanced PMPM payments will be payable.
How can Beacon help?
The Practice Transformation team will continue to provide you support and guidance throughout the CPC initiative. QIAs will serve as a communication channel and can help facilitate questions through CMS as the contracting phase progresses. Once the program begins, the Practice Transformation team will be available to your practice as an advisor and coach, (in a manner similar to the Beacon program) with curriculum and resources to help you in your work. Details of what this will look like will be outlined in your RMHP contract. RMHP will be the contracted CMS partner to deliver practice transformation support to participating CPCi practices on the western slope. Your practice may receive offers from outside quality improvement companies as well. We hope to have the opportunity to continue working with your practice throughout this exciting initiative.
